Designing forms is always a pain, but this website seems to be a good fix

Having the digital Sony become our primary camera has made me forget what good pictures our old Nikon SLR takes. It really makes me want to get a D70.

If the first thing you think of when getting pulled over is “I need to get a picture of this” you are seriously hooked.
